We are delighted to announce that Taurus Healthcare, in collaboration with Herefordshire and Worcestershire CCG, has been shortlisted in the Partnership of the Year Category of the National Link Worker Day Awards.

This award recognises an outstanding collaborative social prescribing partnership. Taurus Healthcare/Herefordshire General Practice has social prescribers in all five of the county’s Primary Care Networks. The shortlisting is for our development of a single approach to understanding patient activation levels across all Social Prescribing teams in order to better support patients and tackle health inequalities.

The winners of all categories will be revealed at a ceremony taking place on Thursday 19 May at Excel in London. We congratulate all of the teams who have been shortlisted, but of course will be rooting for our team to win!
