What is Shared Care?
Shared Care is a collaborative agreement between Taurus Healthcare and your GP practice. It allows both services to share relevant information about your care to ensure you receive safe, coordinated support. Because different providers often use different systems to record patient details, the Shared Care Record helps bridge that gap, giving your GP a clearer understanding of your treatment history and helping them make informed decisions with you. 

Transitioning to Shared Care
When you're ready to move into Shared Care, we’ll send a Summary Letter to your GP. This will include a link to your personal record and full details of your consultations and medications. The letter will also outline:

· Your name, date of birth, and NHS number

· Medication details: names, brands, formulations, dosage, frequency, and start dates

· How long your dose has been stable and the benefits you've experienced

· The date your care should be transferred

· Any ongoing monitoring requirements

· Information on how to refer to Taurus Healthcare if needed

 

Once the Shared Care Agreement is confirmed by both Taurus Healthcare and your GP, we’ll issue a final four-week prescription. After that, your GP practice will manage your prescriptions going forward.
